FAQ about Membership and Prices
How much is a membership?
$124 per month for age 50 and over
$104 per month for age 25-49
$104 per month for age 18-24 without a parent enrolled
$39 per month for age 6-24 with at least one parent enrolled
Members under 18 require at least one parent as a member
There is a $99 enrollment fee
What does my membership include?
An annual physical and up to 5 additional visits (in-person or virtual). Additional visit are billed at the low price of $40. A patient portal for simple, non-urgent questions that do not require an in-person or video visit.
What happens if I need labs drawn or need medical care out of your office?
When you are my patient labs and X-rays or other imaging studies are covered by your insurance as they normally would be. I draw blood in the office and send lab specimens to either Labcorp, Quest, or Sunrise depending on your insurance company. If you prefer to pay cash for lab testing I have an excellent negotiated rate with Sunrise labs. Some HMO plans require a referral from an in-network doctor (not me) to see a specialist. Otherwise, as my patient, you see can see a specialist as you normally would.
What are your office hours?
8:00 am to 4:30 pm Monday through Friday. I close at 12:30 pm the first Friday of each month. The office will closed for some holidays including New Year's Day, Memorial Day, July 4, Labor Day, Thanksgiving, the day after Thanksgiving,Christmas Eve, Christmas Day, and New Year's Eve.
What about after hours access?
If you have an urgent matter after hours you will be able to call my cell phone number, leave a brief message and I will call you back once I receive the message. For your safety, if you are having a medical emergency call 911.
What services performed in your office are not included in the membership?
Rapid Influenza test $20
Rapid covid test $10
Quadravalent flu shot $25
Hepatitis B vaccine (child dose $27.50, adult dose $55)
Hepatitis A vaccine (child dose $32.50, adult dose $65)
Tdap vaccine $45
Prices of tests and vaccines may increase if costs of vaccines or tests increase. I may decide to change vaccine services if demand is low.
You are under no obligation to purchase vaccines from me. Many vaccines can be obtained at pharmacies that bill your insurance directly.
Do you stock other kinds of vaccines?
Not at this time. If there is another vaccine you need I will give you a prescription for it and you can get it administered at a pharmacy or travel clinic. In general, pharmacies in our area that administer vaccines will bill your insurance directly for them. Rabies vaccines must be obtained from an emergency room.
How many patients will you be accepting?
My practice will be limited to approximately 450 patients. This may sound like a big number but it is actually about less than 1/3 of the number of active patients in my previous traditional practice. Scaling back on patient volume is what makes the enhanced access possible.
